Re: Request for Bids - Survey - Children with Disabilities in Early Childhood Settings

In order to determine which childcare providers include children with disabilities in their facilities, a provider will research the early childhood education providers in the state of Florida using resources from several state agencies (e.g., Department of Children and Families, Agency for Workforce Innovation and Early Learning Coalitions) and develop a method of collecting and entering information into a data base in order to retrieve this data for current and future use. The project will be evaluated through the completeness and usability of the data base and the information gathered and reported.

Statement of Work:

1. The provider will identify all early childcare providers across the state, both public and private and develop a listserv which encompasses all of their current email addresses.
2. The provider will assist the CD/ED task force and CD/ED program manager to develop the questionnaire to be used to gather information about the types of disabilities and special needs children in Florida have and in what settings they are being served.
3. The provider will disseminate the questionnaire to all early childhood providers, and follow-up appropriately to ensure questionnaires are completed.
4. The provider will develop a database in order to collect and analyze the data and have the ability to update and continue to use the database as needed.
5. The provider will analyze the results of the surveys and provide a report detailing the findings which will produce a count of children with disabilities in early care settings (both public and private) in the state of Florida.

Expected Outcomes:
The provider will compile a detailed count of children with disabilities birth to age five in childcare facilities throughout the state of Florida. The provider will establish a database of all early childcare providers contact information and the database will be able to be updated for future purposes. The provider will produce a final report that includes data from the survey detailing the types of disabilities being served and the numbers of children with disabilities in each child care setting.

In your bid proposal please include:
1. A narrative and work plan indicating how the work will be accomplished
2. A description of qualifications and experience with similar work completed
3. Any evaluations or descriptions of past or current projects similar to the functions of this project
4. At least three references from past jobs of similar work
5. A bid and budget narrative to accomplish the proposed work
