The nomine for this award must meet the following Criteria:
• Demonstrated advocacy activities in keeping with the spirit of Idelio Valdes.
• Demonstrated participation in the development of partnerships to facilitate community integration, employment and inclusion.
• Has enthusiastically embraced the community and encouraged others to do so.

Nominating procedure:
1. Complete the attached nomination form providing as much detail as possible including specific instances and circumstances where the nominee meets the above criteria.
2. Return the nomination form by 2/19/2010 to mikec@fddc.org or fax to (850) 922-6702

Award Time Frame:
• Deadline for returning nomination forms – 2/19/2010
• Selection completed by 2/26/2010
• Recipient notified by 2/28/2010
• Presentation of Award on 3/16/2010 at the Annual Developmental Disabilities Awareness Day.
